• File size limits configured
• MIME type whitelist (validate magic bytes, not just extension)
• UUID filenames (never use original filename for storage)
• Multer error handler middleware
• Virus scanning for user uploads (ClamAV)
• Rate limiting on upload endpoints
• Cleanup strategy for orphaned files
